For your convenience Baird Health Center has provided Release of Information forms to use for either requesting information to be released FROM Baird Health Center to you or for you to request information from you or someone else such as your health care providers to release information TO Baird Health Center. The Release forms are necessary for transfering information to and from the Baird Health Center and you. You can then Fax your signed and completed release request by sending it directly to the Baird Center at 603-526-3453. Please include your written signature, date of birth, and the SPECIFIC information you want released. Be sure you are using the proper form for your need.
